during a back massage you get undressed as much as you feel comfortable. obviously you will be laying face down, but dont worry, the masseuse will not be in the room...you can wear anything you want on your bottom, but on top they generally want nothing there...if you're not comfortable with that, you can keep your bra on, however they may have to unclasp it so it does not get in the way!
As for length...all salons/day spas offer different length massages/different types of massages for different prices. I generally choose a 90 minute massage because they are VERY relaxing!!

you take off your top and bra then lay on your front and get covered up with a towel
they leave you on your own to do that so its no big deal
