Not unless it's made into something sexual. Even then, most children are confused about something being meant sexual when the adult knows what they're doing. When my sister was younger she had some bad bowel problems and needed suppositories. She had painful bowel movements and my parents would tell her "this is going to feel weird but this will help you feel better when you go to the bathroom." She understood that it would help her and it was treated as a medicine and it was fine.

it is always kind of awkward when you have to do things like that to a child, even one of your own. My son was born with kidney problems and they post pond him getting circumcised until his surgery he would have to have when he was a year old. It was really weird cleaning him, at first, but it has to be done. Then I had a little girl...I thought a boy was bad, its not much fun cleaning a messy diaper on a little girl, my husband was so uncomfortable with it, he would not change her diaper if she was stinky, he said it just did not feel right! so the feeling is totally understandable! not that it is sexual, just uncomfortable!
