Make him watch House. that way he can see how all these odd diseased people come in cause they dont know whats wrong with them. AND THEN maybe he'll consider going.

Tell him that the pain down the arms could be indicative of a heart attack.
If the pain in his back is that terrible, I'm sure there is something that the doctors would be able to be to do to help him manage the pain.
Sometimes people just don't realize how bad they felt until they get help.
sounds like it could be herniated disk. when I did mine the pain shot down my leg but that is because it was in the lumbar section of my spine a cervical or mid spine herniation can cause pain in the arms. I would have this person get it checked out ASAP because it could be something that requires an operation to repair it
