Might be herpes. Next time it flares up see your doctor and they can take a swab. If it is, they'll be able to tell you whether its the sexually transmitted kind or the cold sore kind.

Same thing that causes acne anywhere else on the body. Clogged pores. I think most people get acne down there - not a big deal. Preparation H can dry it up, though, and don't try to pop them because it hurts way too much and will probably go away on it's own.

Could be acne, but without seeing it (and no thanks) I can't rule out a sexually transmitted disease or even keloids. If this is a new problem, you might want to have a doctor evaluate it.
