Question: Initially, the medics can assume implied consent as is appropriate of any unconscious person. But what happens if a pt denies medical help, inside of the box en route to the hospital? Can you drop them off?

If they are AAOX3 let them out.

I would think that the best idea is to first-explain why they are in the ambulance in the first place and the risks associated with denying medical care. And of course, having the patient sign the forms denying medical care. If they sign the form, then you would probably just have them call someone to pick them up from the hospital...at least you are covered if you do everything you can and have their signature-this relieves you from any responsibility should something happen and they attempt a law suit.
