Call the insurance toll free phone number and they will tell you how to do it. Also, be sure that the new doctors will take you as a patient before calling the insurance company. Call the doctor's offices and ask if they are taking new patients.

Dont you usually just say your swithing over please send your records to...the new doctor. Call your old office and the new and see if they except new patients, they usually do. I think you tell them your insurance when you fill out a form when you go there.

1) contact your new prospective MD and see if they are
accepting new patients. If you are in a PPO you may want to check you manual for participating doctors.
2) If you have important medical history, you may want copies of your medical records sent to your new doctor. There may be a fee for copying records. Most office do not allow originals to leave the office. The records are the property of the physician, The patient has a RIGHT to access under state and federal laws.
