See a chiropractor.

Place a hotwater bottle between your back and the chair you are sitting in. Spend time in water to relieve pressure on your back. Gentle walking everyday.

try physio. i have had back problems for the past two years due to a prolapse and its been horrible. i dont know how you have managed for 18 years. i really feel for you.

go to a registered osteopath sometimes you can get a referal fron nhs,
if not go private get them to access your spine and bones and they will tell you if they can help.
i had traction for slip discs and it cured my back.

Take up Yoga, It may not cure it but it will make living with it a lot easier. Mine back only gets me about once a year when I forget and bend the wrong way without thinking. Cat, Dog ,Cobra, Swan, positions done nice and slow , really helps me.
If it continues have you considered a TENS machine. Google it or.

There used to be a product called 'Bella Donna plasters'. They consisted of a plaster, about 8 inches by 4 inches, with bella donna on it. You stuck this on your lower back and just left it 'til the pain went. It gave marvellous support and took away most of the pain. Unfortunately, the EU banned them cos bella donna is a poison. However, it was really the plaster itself that worked cos of the support it gave while your back made itself better.
There are no plasters of that size available on the market and one day my back was so painful I knew I had to do something. Have you ever heard of 'Contact'? Its a kind of sticky plastic sheeting on a roll (for cupboard doors, etc). I cut an 8"x4" piece and stuck it on my lower back. It worked! Not as good as the plaster but well enough to give relief. Try it. Buy the heavy duty one. Cut a few holes in it to allow your skin to breathe. Hope this helps. Painkillers over a long period are not good for you.
