How do health insurance companies know you smoke?!

Because you told them and promised you told the truth. In return for that honesty they will provide your insurance. Besides they read your posts on the internet.

Some just ask others do drug screenings which includes nicotine.

Insurance companies usually ask you on a questionnaire if you do. Depending on your age they may do a health screening on you which will include a nicotine panel.

If you provide a urine sample, yes, nicotine will show up.

You either tell them that you are a smoker or not. So, why not just lie? Because you will still be paying your premium and when you have a claim, they will not pay because the doctor will discover that you smoke.
They don't run any tests up front. But lets say you pay $400 a month for 2 years and then get sick from smoking and have to be hospitalized. You don't get your money back and they won't pay for your treatment. It would be better to just tell them and pay the higher premium.

How is telling someone they shouldn't smoke, "pointless?" because its not pointless, your actually helping someone by making them realize what their doing to their body.
Asswipe, go fill your lungs up with smoke, cause sooner or later they'll be as dark as your ****.
