Tuition varies dependent on how prestigious the school is (usually ranging 15,000- 35,000 USD/year). Most schools offer 2 years of basic sciences in the Caribbean then the last 2 years are offered at US hospitals where you'll be doing clinical rotations. You have to check with your state medical board to see if the school is approved for licensing; otherwise, after graduation from the school, you won't be able to practice in the state that you want.
