You have to check with them on this. I had a schedule and approved surgery to remove a tumor and the insurance canceled it on the day of the surgery claiming it was cosmetic...

The stall only lasted one week after some fighting between me and the surgery group...

I think PART of your answer is: Is the preponderance of evidence would be why and how important this surgery is. Is your life without it so grossly impacted that you can not sleep and work properly?
