I think he tries to hold it in.. because he never seems to go when he needs to.

"holding" is very common with different levels of constipation. Constipation can lead to painful bowel movements, which in turn lead to holding. (viscous cycle...) Small hard bowel movements are a good indicator. Then even diarrhea can result because that's all that can get through. Obviously there are other possible causes and if this is really severe I would recommend an exam by his Doc, but constipation even though it sounds conterintuitive, is the most common reason. Try a stool softener and scheduled bathroom breaks with the intent of BM's.
