Yet another medical case scenario for you guys...?!


Question: Yet another medical case scenario for you guys!.!.!.!?
A relatively fit 50 year-old female is hit by a car!. She has sustained bilateral open fractures of the tibia and fibula!. She was taken to the local trauma unit which was 150 miles away from her home town!.

Any comments regarding the management or complications when dealing with such a patient are welcome! Www@Answer-Health@Com

Well first off you have to be worried about your ABCs!. Seeing as she is getting on in age yield have to keep a close eye on her, but if shes normal size and fit its a bonus!. If her airway is patent and breathing normal, your next biggest concern is blood loss!. Open long bone fractures can bleed like the dickens!. As soon as possible, probably after giving her some fentenyl or other analgesic for the pain, the ambos should be getting one if not two large bore cannules in her and depending on her visible blood loss and blood pressure, if low, pumping the plasma!. If everything else remains stable till they get to the trauma centre she will be needing a prep which would check for any other assessory wounds and neurological impairment as well as to see if there is nerve damage in the arm and leg!. Then surgery, starting with large amounts of cleaning and flushing of the wounds, open fractures on roads are very dirty and infection havens!. Probably will need some ORIF or other fixation of bones and a long stay with physio in the hospital!. Hows that for a basic overview!. :)Www@Answer-Health@Com
