I am not a doctor, and I have not ever even worked for an oral surgeon, however I have worked for orthodontists all of my life and have seen a lot of patients go through it during orthodontic treatment!. So i will tell you what I know!.

Orthognathic surgery is used to correct jaws which are severely disproportionate in size to each other!. Sometimes it is to reduce the length of the mandible (lower jaw!.) Sometimes it is to reduce the height of the bone in the jaw so the patient doesn't look like they have such a "long" face!. These are all cases where there is so much discrepancy in the proportions of the jaws that orthodontics alone will not solve the problem!.

When the orthodontist sees that his patient needs surgery, he has a consultation with the patient and discusses all options with the patient!. if the patient agrees to the surgery, then the doctor refers the patient to an oral/maxillofacial surgeon!. The surgeon looks at the Xrays and plans the surgery!. He writes a letter to the orthodontists and they keep in contact during this time!.

Just before surgery, if the patient already has braces, tiny surgical hooks are placed on the braces!. These can be tied onto the wires just near each bracket, or sometimes a special archwire is made with hooks on it!. In either case, the hooks are so that when the surgery is performed the surgeon will have hooks to secure the wires to for securing the jaws into place!.

During sugery, the jaw bone is cut, a precise section of bone is removed to bring it to the desired size, and it is re-aligned and re-set!. Small plates and screws are used to hold the bone!. These are permanent and will always be a part of the patient!. Afterwards, during the last part of the sugery, wires are tied to the hooks to keep the jaws closed while they heal!. this can be in for at least a week!.

After surgery the patient is put into critical care (ICU) for just a short time to watch him closely and make sure the airway remains clear of blood and fluids, and to maintain vital signs!.
The patient usually does not spend much time in there and is moved into a regular room soon after!. Healing happens very fast and most patients I have talked to said that they were surprised how little pain they were in!. Everyone i talked to were happy with their results and were glad they had it done!. The change in appearance of the face is quite drastic, in a positive way!.
