Immunization record required for a healthcare course, how do I get them?!

Question: Immunization record required for a healthcare course, how do I get them?
I'm signing up for a CNA course and an Immunization record is required.

I need documentation of Hepatitis B shots three of them.

Measles, Mumps, Rubella 2 shots

TB/PPD skin test

Varicella/Chicken pox either Immunization dates, Positive titer showing immunity, Medically documented history of the disease.

I'm 32 and mother isn't where to find my immunizations. My old (he's at least 80yrs. old) pediatrician is still working, but his office has never used a computer, I don't know if he's gonna have the records.

Would the health department have the records, no I never got shots there only at the doctors.

I did have chicken pox, but I'm unsure if there was any medical record of it. Just not sure where I go to get this information anyone have some suggestions.

Answers:

Doctor's offices and even your old public schools isn't going to keep records back more than about 10 years, so since you're 32 years old I seriously doubt any of those places will have your records.

There are ways to prove that you've had these tests. Go to your health department. The health department can do a titer test which takes about two weeks to get the results back, but it will prove you've had your MMR shots and that you've had chicken pox. You should do it as soon as you can because it takes two weeks, you don't hold up your class because you have gotten this done.

Also, at the health department you can get your hepatitis B shots you haven't had these unless you've already worked in a healthcare environment.

Though make sure there's money in the bank, the shots you need (there are more than you've listed here) and the titer tests will cost some pretty good coin.

My sister is currently enrolled in nursing school to become a nurse practitioner, this is what she had to do when she enrolled in her first healthcare course.

You could try the health department, but unless your doctor put the information into a state vaccination registry, they probably won't have any records. But it wouldn't hurt to ask. Since your doctor is still around, ask him. He may have the records, and a record that you had chicken pox. There also may be a copy in your school record. If you do not find the record, you can get blood tests for immunity to measles, mumps, rubella, and chickenpox. Unless you had a reason to get the hepatitis B vaccine in the past, you probably did not have it; if this is the case, just get the 3 shots, and a blood test 4-8 weeks after the last one to prove immunity. The TB skin test is needed shortly before you enroll, so an old record of TB tests would not count (unless you had a positive one in the past, in which case you should not get more TB skin tests).

I got mine from the high school I attended not sure if they would have yours tho since they toss em after a certain amount of time. Other than that probably the place(s) you went to get your shots at.
