Question: Help: Letter asking current doctor to discharge me from his case register (changing doctors)?
Hi everyone, I have been seeing my current (private) psychiatrist for 12 months but have decided see someone else more local instead. Just wondering if anyone would be so kind to help me compose a formal yet "nice" letter asking this doctor to discharge me from his case register? I have no idea what to say and what is/isn't appropriate. Here's more details:

- I was assessed by and subsequently received my diagnosis from the current psychiatrist.
- And since I've been having monthly appointment with him (for prescriptions and general chat)
- He likes me and is very pleasant but I don't feel he is someone I connect with
- I recently found another psychiatrist who I really like and is local - of course he won't take me on until i've been discharged from the previous doctor.

So my question is...in the letter, do i need to give reasons? there's nothing wrong with my current psychiatrist, he is fine but i'd prefer to see someone down the road who i feel more comfortable with. i don't want to be rude and in case things don't work out with this new one, i'd like to have the option to go back to the old one.
