What is a good, tactful method to offer your hand for a handshake when one has painful Rheumatoid Arthritis.?

Without prefacing the handshake, with: "Oh please don't squeeze my hurting hand."
And if I don't offer my hand, I'll seem cold and rude.

I have arthiritis and am a little gun shy when it comes to shaking hands until I found out about the "arthritis hand shake". Extend your hand towards the person palm facing down, thumb pointing towards the left, rather than up. They have to turn their hand to meet yours, and can't exert as much strength when they shake your hand. Check out the link below. Best wishes.
