shoulder/elbow pain in right arm?!

K so this is for my boyfriend.. Hes recently been having terrible pain in his shoulder that has spread to his neck then back down towards his elbow in his right arm. At first we googled and thought it might be a rotator cuff injury (he woke up one day with a dislocated feeling in his shoulder) but now the pain seems to be wearing off his shoulder and transcending down to his elbow and the muscle between his elbow and shoulder. What could be wrong? Hes been taking muscle relaxers, tylenol, rubbing lotions on like banalg and bengay yet nothing seems to be improving the pain for very long and just today tried some lidoderm patches. hes a man and basically refuses to let me take him to the doctors therefore havent even pushed that option but there seems to be nothing improving with what were doing at home. What can I do to help his pain? I already give him massages and try to ease the pain cause it actually feels like there is a knot under his arm (not his armpit just under the muscle above the elbow). Any input would be greatly appreciated.
