It is possible that he had a middle ear infection, which built up pressure behind his eardrum until the eardrum ruptured. Or perhaps he had some water trapped in his ear originally, which led to an outer ear infection, which can lead to some bleeding (although rarely "a load of blood"). If he is still having trouble with his ear, he definitely should get it checked out by a doctor. If his eardrum is still ruptured, he might need to have it surgically repaired if he wants to avoid permanent hearing loss in that ear. If he has a chronic outer ear infection, it can spread to the face and eventually to the brain, especially if he is diabetic. This can also be treated and prevented. There are also potentially more serious causes for his symptoms, such as a cancerous tumor, or a non-cancerous growth called a cholesteatoma, which nonetheless can eat into his middle and inner ear and cause serious permanent damage.
Encourage him strongly to make an appointment with his primary physician sooner rather than later.
