how do i retrieve my accident and emergency (A&E) records (NHS)?!


Question: How do i retrieve my accident and emergency (A&E) records (NHS)?
Recently i was admitted to the hospital throught emergency.
When i wasdischargedd home they gave me theletterr which contained my diagnosis and treatment which they gave me (all meds etc).
Hospital stuff told me what they would send its copy to my GP but icouldn't remember theaddresss of practice where i was registered or my NHS number so they told me to give it to my GP in person.
I just moved to UK and never been to my GP before.
When i came back home i went to a GP in my praactice and i gave him the letter from the hospital.
I didnt liked that doctor. So next time (a few days later) i booked an appointment with another doctor. But when we started to discuss the accident what happened to me he wasnt able to find any recors of meds or anything about it in the practice system. Couse previous doctor havent writen them down. Previous doctor also lost this letter from the hospital which i gave him.

I was wondering how can i retrieve records about my accident from that hospital where i was treated. They dont know my NHS number. But when i was there i gave them my adress and my name. Is it posible to ask them to send me that letter again? do they even keep the records?

Answers:

The hospital will still have a record of your medical reports. Call or write them and tell them you need a copy of your records from (specific dates). Be sure to include your SS#, birthdate, full name, insurance policy you used if any . You need to call the records department of the hospital, they will send you a paper to release your records to you, it must be signed and sent back to the hospital before they can send you any records. There also may be a fee, be sure to ask the fee when you call and when you need to send the fee. It would also help if you know the name of the Dr who treated you but not absolutely necessary. You will only get copies of your records, hospitals & Drs keep the originals. I've got my records from different Doctors many, many times when I was sent to a different Dr. Your GP can also call the hospital and ask them to fax your records to him. I don't know if they will, most will but some won't. But you can get your records by the above means.

When you change Dr's and surgeries you can register and be seen straight away but it can take some time for your records to follow. Some are computerised and some are still handled by hand but the system is not as matched up and efficeint as you would think it is.

If your new Dr needs more details and a copy of the letters he can get copies from the hospital if he wants them. You yourself cannot get access unless you jump through hoops but the new Dr can simply write in and get copies if he feels it is important enough to follow it up.
