my friend who is a 20year old male is throwing up yellow fluids and is having severe pain in his lower left back. his girlfriend thinks its his appendix but I dont know

If the pain is severe and he's vomiting and the reason is unknown, then definitely have someone get him to the ER or call an ambulance if no one is able to drive him themselves.
