Take a look at this case study: Under a general anaesthetic, the dislocation was reduced by longitudinal traction, ulnar deviation and direct pressure on the bony prominence as the wrist was brought into neutral position. Anatomical reduction was confirmed under fluoroscopic examination. Two smooth K-wires were drilled to hold the reduction, one between scaphoid and lunate and one to hold the intra-articular fracture fragments of the distal radius. A below-elbow thumb spica cast was applied postoperatively. Pins were removed after 6 weeks and immobilisation was discontinued at 8 weeks. Physiotherapy was initiated to regain the strength and range of movements of the wrist. At 16 months follow-up there was no clinical or radiographic evidence of carpal instability, avascular necrosis or arthritis. There was 120° of flexion/extension and 140° of pronation/supination at the wrist and he had resumed his job as a delivery assistant in a store.
