My boyfriends knees give him constant pain, what could it be?!

Question: My boyfriends knees give him constant pain, what could it be?
He's 23, and is very active as he cycles everywhere.

For a few years now he's had extreme pain in his knees constantly on a pain scale of (10 being the most pailful) 4 all the way up to 10 sometimes.

He saw a doctor who referred him to a specialist, they did a CT scan on him which found nothing.

My boyfriend thinks he's damaged both knee cartilages due to cycling but nothing could be seen on the scan.

He's made his seat higher on his bike to avoid straining the knees, and also booked a consulation for some physio (recommended by the doctor) but he says this won't help him as the physio will only make him do excersises like cylcing and swimming which he does anyway and what he believes caused the pain in the first place.

Can anyone tell me what this is ? or how it can be fixed? (without having to resort to quitting cycling - as it's also something which effects him even walking up the stairs now, and he shouldn't have to give up cycling or long walks at aged 23)

Thank you

Answers:

if he is in that much pain there has to be a physical cause, just because the doctor didnt find anything doesnt mean he hasnt got damage of some kind to his knee.
doctors often get things wrong and send patients home.
he should insist that he needs further investigation, and dont take no for an answer.
in the meantime he could pay to see an osteopath, he might have an opinion on what is wrong, they are involved with a lot of sport injuries.

I think he should se a doctor. It could be genetic..?
