There are a few companies dedicated to providing a 'domicillary' service, meaning they come to your home with special equipment to do a full sight test on you. They can also sort you out with glasses if you need them. If you genuinely can't leave your home for medical reasons (usually problems with mobility but as long as your GP has agreed you can't leave your house unaided), the service is free.
