For you, the position that your eyes need to be in when you look at something up close causes strain on your eye muscles. The prism relieves strain by letting your eyes use a position that is more natural and comfortable for you. Hard to explain without going into optics, but prism just changes the way light is directed.

Your muscles in your eyes don't focus exactly where they need to, so prisms are put into your glasses to direct light in the right area. I diopter, you have 1/2 in each lens ('base in' is the direction the prism in placed) is very mild.

The prism is in your glasses for the exact reason the optometrist above me said. It's a muscle issue, but don't worry, it's not super uncommon. Be happy you went to a doctor who found the issue.
The only thing I really wanted to add is your prism is
"half a diaopter Base In" which means if untreated, your muscles around your eye would want to "pull" your eye outward, so the prism acts like a little muscle to help pull the eye back in. That's part of the reason you used to get headaches... just like your muscles ache after a good hard workout, your eyes can give you a headache if you try to see things without your glasses because they are strained without them
But your prescription isn't too strong so no worries!
