Faced with someone with unexpectedly low accommodation, (ability to refocus for near) I would do a cycloplegic refraction, using a stronger version of the drops used just to dilate the iris!.
If this hasn't been arranged before-hand, and driving or other factors means it can't be done at the first visit, I would set up a second appointment!. In which case I wouldn't need to dilate at the first!.
But cycloplegic drops will leave the pupils large and the reading vision very blurred for hours!. The distance vision might also be blurred depending on whether or not hidden long-sight (ciliary spasm) had been shown up by the drops, the reason for using them!.
