Is it possible to have eye convergence insufficiency and eye divergence insuffic!

Question: Is it possible to have eye convergence insufficiency and eye divergence insufficiency?
I have been like this my whole life. When I was a baby I had eye surgery. I believe it was to strengthen one of the muscles in one eye. I wore an eye patch for a little bit after the surgery. I use only one eye to read things. It doesn't matter if the thing i'm reading is right in front of me or across the room. If I try to use both eyes, I see double. I sometimes lose my place reading, both words and music. I can only read one word at a time and I often skip words or lines on a page by accident. If I try to hard, I get a headache and eye strain. Usually I just turn one eye off. I can do this voluntarily and I can turn either eye off. Sometimes when I read, I tilt my head, close one eye, and use a ruler or finger to keep my place so I dont skip around.
