In the UK - If you have an eye examination then whoever did the test is required to give you a copy of the prescription. If they didn't, you can phone and ask.

But if this is a report from other tests or examinations it forms part of your medical records. Usually your eye care professional will discuss it with you but if you want to see the report you MAY have to request a copy of your records in writing from the hospital. You can legally request a copy either from the hospital or your GP, but there is usually a charge for administration.
