Prescription of eyeglasses?!

Question: Prescription of eyeglasses?
I want to order Rayban prescription glasses online so I went to my eye doctor for my prescription
All he wrote on the paper was my
OD - 3.00
OS - 3.00
PD 63/61
now when I go to the site to order my glasses its asking for my right PD my left PD my axis and cylinder for both left and right eyes
what do i fill in for those?
My doctor didnt write any of that on the paper.

Answers:

Cylinder and axis is for an astigmatism correction. You don't have that. You just have sphere power, so leave the other spaces blank

Your distance PD is 63 combined , but if they want it separate, put 31.5 for each. If they don't have the .5 available put 31 for each eye, it doesn't really matter for 1 mm.
