If you are a virgin and your hymen is intact, then yes, we can tell that you have never been sexually active. If you have had sex before but are not having it now, then the answer is no, we can't tell. If you had sex last night, we may be able to detect sperm if we look at vaginal secretions under a microscope.
You should not be embarassed about being sexually active or not being sexually active. We have seen it all.
It is VERY important for you to inform your doctor however if you are having sex, it's something they need to know in order to best determine what tests and/or treatments to give you.
Not unless seman is present. But they can tell when them use the speculum to open and see the hymen
