In the US (and most other places) you are legally an adult at 18, so what happens at any doctor's appointment is entirely up to you. No one, not the doctor or your parents, can legally make you do anything.
You don't have to have a checkup if you don't want one. Breast or vaginal exams would not be normal at a checkup in any case, but if you don't want them, say no. If you don't want a shot, say no. All your medical decisions are entirely up to you.
Why don't you call your doctors office and ask to speak with the nurse and ask her what the exam will be like. Since everyone is different, other peoples experiences won't necessarily be the same as what your doctor will do. It's normal o be nervous but knowledge is power, so call and ask that is what they are there for.

You don't need an internal examination at 18 unless you have been sexually active or on the pill and need a pap smear. Your doctor also doesn't need to see your breasts. If you haven't had a check up in a long time he may want to do a breast exam to check for lumps (cancer) but I don't think he will.
