"an i tell the doctor that i prefer no nudeness and blood draws?"

Yes, of course. You're the customer! You can do anything you want. You don't have to even go to the doctor if you don't want to. Why are you going for a check-up? They can't do anything without your signing a consent form, so you need to make sure you know what you're consenting to. They certainly can't make you disrobe.

"Will they accept my question/comment?"

Most likely. If you refuse a blood draw, they won't be able to do blood based tests, but it's your choice when or if to get tested. Here's the thing, though, if they don't accept your terms, guess what? You can leave, and find another doctor who will! You don't have to give your business to doctors who won't let you make your own decisions.

Frankly, there's no such thing as a routine checkup. It depends on what you want checked.

Is this a regular physical? If so they will take your vitals (BP, pulse, temp, height and weight) and examine your heart, lungs and abdomen. A pelvic exam is not routinely done at a physical- the doctor will send you to a gyn doc for that. A blood test is usually done to make sure you are not anemic, don't have high cholesterol, or diabetes. A urinalysis is usually done too. You can refuse these tests however they are for your benefit and the pain from the blood test will be minimal.

They will go to great pains to keep you covered. But you must be tested, that's how cancer and other problems are caught early. He will have you sit at the end of the table, the bring out metal stirrups for your feet. Then you will have to scoot down so doc has good access. He will then insert what is called a speculum, it will open your vagina so he can see your cervix. He takes a sample of the cells on your cervix, and birth canal. It hurts, but just for a few seconds. There will be a nurse in there with you the entire time. He will probably check your breasts and tell you how to do this at the same time. You need to be checking your breasts every month. Remember this is his job, he looks at tons of vagina's and breasts every single day. It's just another organ that needs to be cared for.
