In the eyes of the law, you're still a minor - so your parents have a right to know what's going on. Now, unless your mother has specifically asked him to check to see if you're a virgin (which if he checks for a hymen, it's unreliable since hymens can break very easily while playing sports or doing simple things like riding a bike when a girl is young) - there is a wonderful thing called:

Doctor patient confidentiality.

If he asks you if you've had sexual intercourse before, and you must tell him the truth when it comes to your womanly health, just tell him to keep it between you. Anything you say to him cannot go beyond the walls of that room.

He will keep it to himself.

If he tells, you can sue him.

very doubtful he will say anything about it. other than asking you about birth control, and using protection. and just my opion , that if your worried that if mom will find out, maybe you should hold off having sex for awhile.

It depends on your age and what the laws are in your state. You can always ask if what you tell him or her is confidential to be sure.

Im not sure but I do belive that a doctor cannot tell anyone anything without your permission

Actually it's quite likely that he cannot tell. It's not like it leaves permanent evidence. He's supposed to keep that kind of thing to himself anyway. I don't know, but I think he has to report it if you have a sexually transmitted disease, but again, I'm not sure..

No he/she will not tell your mom. Your mom doesn't even need to be there. You might what to tell her you would rather go by yourself. It may by hard for you to have a frank conversation with your doctor if she is present.

i think he can tell her if she requests to know. my mom did this to me!!! surely to goodness you have a mom that wont do that, or doesnt even think about that. good luck.
