you can't really tell, there are silent orgasms and noisy ones, ones with lots of twisting and twirling, heavy breathing, etc.
the quiet ones are impossible to tell - sometimes the girl only takes care of the guy's needs - the you both know it was a mercy thing.
i have no idea on how a woman could fake one i mean come on. The breathing, moving around, some that squirt i mean come on how can they do it.
