OK, first of all, no-one 'becomes' transsexual; if you're not BORN transsexual, you're NOT transsexual.

Now, if your "friend" wants to take hormones, he must first see a psychiatrist with experience in gender issues; this is to establish that he IS transsexual, and not mentally ill, fantasising or delusional. It's very unlikely that any doctor will prescribe hormones to someone without that diagnosis (here's a hint; 'I'm gay', and 'I want to be beautiful' WON'T cut it)

Assuming he gets a diagnosis, he will then have to either have therapy for three months, or live as a woman,24/7, for three months before he can get a prescription. (I know there are 'ways' to get hormones without prescription; but Russian roulette is a safer, quicker and less painful way to die than taking hormones unsupervised, or bought on the 'black market')).
The effects; there will be some breast development; how much depends on his age, his genes, which formulation he's taking, and what dosage. It's very unusual for breasts grown this way to exceed a B cup.
His body hair will reduce, but it will take a LONG time for that to happen; there will be NO effect on facial hair (that will have to be removed by electrolysis, or laser treatment).
After a long time on hormones, his body fat will redistribute to give him curves in the appropriate places; as with breast development, this will vary, and won't be spectacular (unless he's pre-pubescent, which is unlikely).

Incidentally (contrary to popular misconception) female hormones will have NO effect on a male-developed larynx, which means his voice won't change, unless he has a lot of speech therapy.

The main disadvantages are; sterility and impotence (both permament, after a few months), loss of libido, weight gain, a much higher risk of DVT/pulmonary embolism (usually fatal, hence the need for medical supervision), and an increased risk of breast cancer.

After one to two years - depending on where he goes for treatment - on hormones (AND living as a woman 24/7), he may be considered for genital surgery. Which is painful, and of course permanent; you have to be VERY certain that you want it.

Still want to do it?
Talk to your doctor.

He will start to grow breasts.

They used to think giving certain criminals female hormones would reduce violent behavior; it didn't work, but the guys did grow breasts.

I'm not sure if his beard will stop growing; he still has his testicles. It sounds like he really wants a sex change op.

He will probably grow small-ish breasts, though his body will never acquire true female curves.

He will still grow facial/body hair though. That's why so many transexuals go through electrolysis to get the hair removed.

He may become somewhat more feminine, but everyone will always be able to tell that he is/was originally male- the voice and body structure will always be a give-away (at least to those of us who have seen a lot of tranny's).

Major disadvantage: Adding estrogen hormones increases the risk of cancer.

How young is your friend? Being gay doesn't mean that one needs a gender change. A man can be a feminine man without turning (or attempting to turn) into a woman. Besides, other gay men don't always go for transexuals - they MIGHT, but the gay men I've met like boys who look like boys. Heck, that's why they're gay!
