I need help about to start hrt, i am a mtf?!

Ok so i am 24, i have know since i was atleast four i am a girl. Problem is nature thought a little joke would be funny and I am not physically who I am mentally. I was seven When i realized i wasnt a girl, and i was devasted, yeah i know 7? I never payed attention to my downstairs especially back then. The shock came when me and my sisters were playing dress up and my dad came home, i was exicted to show him but the look on his face even scares me to this day.My whole life have been picked on, the center of jokes and continusly called gay for being femmine, eventually i learned how to hide my true self, nd since then i feel like an actor must ebvery day i have gotten up since.I decided i cant live like this anymore and am ready to start hrt. The only problemis i Cant find any doctor in northern michigan that services the transgender community. which leaves me with the option of either, putting the fake mask of deception on again, traveling 4 hours to detroit or self medicate. Is there anyone that knows if there are any docs in northern michigan, cause self medicating is on the bottom of my options list, i know its risky no need to explain thats why its an absolute last option.
