Depends on the girl, some girls are in pain the entire time and some just the first few minutes. Some girls will even bleed, as the hymen usually breaks the first time they have sex. It's VERY important that she is lubricated enough to enjoy it. This means, heavy foreplay and making sure you're hitting her spots, if you know what I mean. Be sure to have some KY Jelly on hand too. Oh, and use a condom. :)
Not necessarily, yes there can be some pain and blood involved in a girls first time, but it all depends on the girl. I didn't have any pain at all. We went for like 2hrs my first time not including foreplay. Just start with lots of foreplay. make sure she is ready and tell her to relax and there shouldn't be too much pain.
She may be very uncomfortable at 1st, which is going to cause a little more pain. Just be gentle with her, make sure she is comfortable. Alot of woman have horrible 1st time experiences because they are so nervous, and scared of what could happen, (bleeding etc. which can be normal). Just try to keep her calm.
