It depends on whether or not they examine you.
If they do examine you, don't be embarrassed, you have NOTHING they haven't seen before, hundreds if not thousands of times.
As to whether it will be painful, they wear gloves and generally would put a little lube on to make it easier to slide their fingers in if they need to.
The gender of the doctor doesn't matter, it's the experience that counts. If you are more comfortable seeing a female doctor, then ask for one specifically.
Painful? No. Embarrassing? Maybe. Male or Female? Whatever you are comfortable with.
