I doubt it but you never know. The device is located high up inside your womb well away from your cervix. Even rough sex would not dislodge it. Usually if they move it is within the first month and because they are not inserted in the correct position to start of with. It could just be your womb lining had built up slightly over the months despite the Mirena and your having an intense orgasm has triggered this bleeding. Probably the strings were tucked up inside your cervix and have now dropped down slightly.
You are doing the right thing seeing your Gynaecologist for a check up. If having "rough sex" with your partner causes bleeding sometimes you should get that checked out, a penis is not hard enough to damage the surface of your cervix enough to make it bleed. Any bleeding would be coming from within your womb I imagine (again small amounts of uterine lining being shed).
