Severe infection sets in eventually and the woman can get very ill and even die if left untreated. This would have happened quite commonly in years gone by before the advent of scans and D&Cs (operation to scrape the inside of the womb out).

I had a "missed miscarriage" at 17 weeks gestation around 24 years ago. My baby died at around 13 weeks. I had severe food poisoning at the time and when that subsided my morning sickness had gone and I thought this was natural. A week later I started to bleed a little but my family doctor just advised bed rest and he said a scan was not necessary as I would most likely be fine in a few days. So I lay in bed for nearly 4 weeks and the bleeding gradually got worse. I called the doctor out 2 more time and each time he said "don't worry it will pass".

Finally a different doctor called and was horrified I had not been referred to hospital. She arranged for a scan and it was discovered I was carrying a foetus that had died about 4 weeks earlier and I had to have a D&C to remove the dead foetus and 2 lots of antibiotics to deal with resulting infection.

I reported my doctor for negligence as he failed to send me for a simple scan which would have avoided all this trauma. He got a written warning.
