I know how you feel. The best thing you can do is just make an appointment and get it over with. If your doctor is a male and that makes you uncomfortable, you can look around and find a new girl doctor just for a pap smear. Make sure you're showered and clean.
My first time getting one, I had a female doctor and I was not very nervous. My second time after I moved, my new doctor was a man and I was extremely nervous about him being down there. BUT when he did it, he had one of the female receptionists come in the room while he did the pap smear, which made me more comfortable for safety reasons.
