There should be one "best" set of practices that is available to everyone, and then people can decide what they want to do based upon the overall health and economic factors. But the "best" plan should be available to all - health care professionals and patients alike.

You need a pap smear every so often because you could get HPV any time (especially if you are sexually active). In the end having a pap smear prevents you from getting cancer. So it is in your best interest to get one.
