To be honest, this procedure is very rarely (if ever) covered by traditional insurance companies. It's considered an elective procedure--- classified the same way as plastic surgery. And, since all state laws require people to be counseled about the permanence of sterilization (for both men and women) the insurance companies have no legal reason (and to be honest, no financial reason) to cover it.

The success rates are also suspect.

Many insurance companies will pay for some basic infertility services (like testing), but not in vitro fertilization for the same reason. But, you might want to consider egg harvesting and assisted reproduction instead of the reversal.
