i had one break before--it was a tampax pearl and ironically i got a "forward" email about a month later warning women of the pearl tampons. it took about a 1/2 hour for me to get it out, but luckily, i did. i think it happened because the tampon was still quite dry. i've been using tampons for about 15 years and that was the only time that's happened to me and i've never had anyone i know tell me it happened to them. use in confidence!
