It's not exactly possible. They only way that a doctor could suspect that a woman is sexually active is through the absence of a hymen. However, there are many ways a hymen could be torn as well. The vaginal wall "may" look stretched, but that could be caused by a number of things, including tampon use.
If you are sexually active, it is best that you are honest with your doctor. He/she will have to keep your information confidential (they are not legally allowed to tell your parents) but it will help them treat you best if they know that you are.
