Having just finished working for the NHS after 7 years as a substance misuse worker, I have seen how some people get put on a script and they stay on that script for life, be it methadone or buprenorphine, or in this case heroin.
For many, they go on a programme, and every so often they don't see the programme through for whatever reasons, and they (re)lapse. So I don't think it matters what is given, as long as society, government/NHS keep trying to help those who ask for it and need it. The cost in giving someone who is opiate dependent a script is surely far cheaper than policing them when they go to commit crime or when they need hospital treatment having injected a "bad batch" of heroin, or had an OD, or contracted HIV from shared works, etc etc etc
