1. Educate the pt providing education about the physical basis for the d/o increases understanding & acceptance and decreases blaming behavior.
2. Evaluate & observe for hallucinations, redirect back to reality by distracting pt w/conversation; Early assessment enables evaluation of pt responses to hallucinations and how much time pt focuses on them. It also enables the nurse to assess if hallucinations place pt or others at risk & permits early intervention.
3. Assure pt you will privide safety for him regardless of what the voices say will happen; This provides an anchor to reality and decreases pt fear that harm will occur based on what the voices say.
4. Avoid touching the pt.; Distortion of reality may lead pt to misinterpret physical touch, which along with excessive environmental stimuli can increase anxiety and precipitate hallucinatuions or aggressive response.
5. Administer sntipsychotic meds as prescribed; Antipsychotic meds reduce psychotic symptoms.
6. Teach pt to verbalize fears & describe methods for managing anxiety and stress constructively; Verbalization of fears is 1method of reducing anxiety.
7. Determine when anxiety increases. Stay with pt to ensure safety.; Increasing anxiety often precedes hallucinations.
