The proper way i guess is on your hips, but i wear mine about 2 inches below my belly button ( im 6'5) orif your athletic and wanna show off to the girls, wear them like they do in those calvin klein ads lol

Not too high, because they ride up. At least for me they do and i usually have to move them down a little.
get em up as high as they can go with out squeezing your nuts, at least past your bellybutton
