Question: well one of the characters got shot on the side of his stomach, and he's in the hospital,(will survive),but i want to add a scene where he starts to puke some blood and he needs an emergency operation and all, is that kind of a scenario be realistic?
and if it is, what could be the cause of him puking blood? could the bullet slowly move throughout the body?

yes it is possible
he could have a reaction to the medication he was given at the hospital and bullets often times do move throughout the body and it can be very dangerous...he could also have food poisning
He's puking blood because when they first operated on and then sewed him up they accidently left a scalpel in his side. That thing began to cut other vital organs (which is why the bleeding from mouth) as Xray reveal and so there has to be another emergency oparatoration to remove scalpel.
