He may have strained too hard when trying to have a BM. Most likely needs to increase the amount of fluid that he drinks. If it does not go away in a day or two, or his has a lot of pain, he should see a doctor.

Amoebic dysentery is very unlikely since you did not mention diarrhea.

Blood with diarrhea could be amoebic dysentery.

It sounds like an anal fissure. An anal fissure is a tear in the lining of the lower rectum (anus). They can be caused by large, hard stools, repeated diarrhea and other reasons explained in the link below.

The best things he can be doing if it is a fissure is drink lots of water, take in extra fibre and make sure he goes to the toilet as soon as he feels the urge for a bowel movement. His doctor may also prescribe a cream or suppository to help with the pain and to aid healing. He may also be advised to use a stool softener temporarily to cause the bowel movement to be more loose and thus easier to pass.

Fissures are not precursors to cancer or any other more serious problem.

Hemorrhoids could be a possibility. If his stool is hard or he's been 'exerting himself' to pass a bowel movement, that can cause hemorrhoids. Eating more fruit and getting more fiber can help with that.
However, there are plenty of things that can cause anal hemorrhaging other than hemorrhoids. Eating anything that doesn't digest entirely can possibly cut you on the way out.
Make sure to check the stool and see if there is blood in that, because that's when you should perhaps consider taking him to the ER. If it was just painful and there was dots of blood on the tissue (maybe a smear if one 'popped'), then have his grandma keep watch on him. It's probably nothing to get worried over. :) Good luck!

It most likely could be hemorrhoids .

Hemorrhoids occur when the diet lacks fibre.

He should have a high fibre breakfast cereal with yogurt.

For lunch and dinner he must have more veggies, and for dessert eat fruits.

Maybe you should have a nice talk with his grandma about nutritional benefits?
