Emphysema is permanent lung damage, usually due to smoking. It doesn't get better but there may be additional asthma or "reversible airways disease" which will improve with treatment and obviously any infections should be treated. Smoking must stop immediately to prevent more damage. Home oxygen overnight helps prevent some of the complications of severe emphysema and improve quality of life.
Rarely there are one or two large cysts which compress the remaining lung and these can be surgically removed and improve function. If overwieght reducing this can also improve lung function and make it easier to move about.
